Good question. Read a 2026 PBM contract. Or better yet, run it through an LLM and ask it just how pass through it is.
The language of the contract gives complete control to the PBM over what fees and rebates are pass through and what they keep
More importantly.
Even dollars that are passed through, ARE NEVER FULLY PASSED THROUGH TO THE PATIENT, if at all.
The employer keeping the rebate is just as bad as the PBM keeping the rebate. It’s the same problem.
Sick patients write the checks for those drugs, particularly during their deductible phase.
Look at reality. If a company has no members getting any prescriptions, there are no rebates at all. So by definition, it’s your sick employees or their families that pay for the rebates.
CEOs making money from their sickest employees is just as wrong as PBMs keeping that cash.
The approach I outlined not only ends rebates, regardless of who receives them, it opens the door on full pricing transparency
